One in 300 people has FH

Stichting LEEFH is the Dutch national expertise center for familial hypercholesterolemia, an inherited condition that keeps cholesterol high and raises the risk of cardiovascular disease, sometimes early in life. The foundation guides families through screening and supports care providers in recognizing and treating FH, from Amsterdam and through a national network of centers.

Two very different visitors arrive at the same website: a family that has just heard the diagnosis, and a doctor who needs a score, a reference value or a guideline in the middle of a consultation.

Two routes through one site

We split the site into a route for patients and family and a route for care providers, with one shared knowledge section behind both. Patients read what FH is, how family screening works and how to request a test kit. Care providers get diagnosis, treatment and the tools they use while a patient is in front of them.

The test kit request is a form with rules in it. It asks whether the visitor has spoken to a doctor and whether DNA testing has already been done in the family, and it stops the request with an explanation when the answer means the kit cannot be sent yet.

  • Separate routes for patients and family, and for care providers.
  • A test kit request form and a cord blood kit request form.
  • Three diagnosis tools: DLCN score, cholesterol reference values and an LDL-C calculator.
  • A center finder with a radius of 25 to 200 kilometers.
  • An English version of the site next to the Dutch one.

The rules inside the request form are the part that keeps out the requests a doctor has to see first.

The result

The site is live on WordPress. Test kits and cord blood kits are requested through the site, and a visitor finds the nearest LEEFH center by typing in a town. The diagnosis tools sit where care providers look for them rather than in a PDF.

The knowledge section brings the glossary, the videos, the scientific guidelines and the Lp(a) information together in one place, so a family and their doctor read the same source. Automations and findability were part of the same assignment as the website.
